operator for the AMASC Meeting in New Orleans. To register for
AMASC and the special events, please complete this form and mail

Please do NOT mail faxed registration forms. Registrations with
payments must be received by March 23, 2006. Requests for cancellations
must be in writing and must be received by Crescent City Consultants
no later than March 23, 2006. There will be no refunds or exchanges
after March 23, 2006. Tickets purchased on site will be assessed
a $3.00 fee.
Tours will be filled on a first come, first-served basis. All
tours require a minimum number of participants.
If this minimum is not met, tours may be cancelled, and refunds
will be returned on-site at registration.
